Super 6 Soccer has public liability insurance and professional indemnity insurance to the value of $20 million. Super 6 Soccer has player injury insurance

Note the injury insurance is very basic cover and it is advisable to use your own private health insurance if you have it.

You will need to put in some detail .Club is Super 6 Soccer, Policy number is  PMEL/0124825. Put N/A for all the association stuff. The address of injury will be at the park you had the injury. Please note you only have 30 days to submit a cliam. 

When you get your claim form- fill in as much detail as possible, get your doctor to fill in his/her section,  scan and email it to admin@supper6soccer.com.au for us to approve.

We will fill in the remainder of the document and email it to insurance agency-  Sports Cover . 

Somebody will contact you to discuss your claim and give you a claim number.

Make sure you keep all your medical receipts-you will need them for claiming.
